We offer one-on-one care and support, development opportunities, and vocational development. When required, we provide community rehabilitation, which in turn increases independence and social integration.

We collaborate closely with your primary care physician to track your progress and treat any symptoms that arise. Depending on your needs, you may receive one or more of the following types of mental health services at home:
Help managing symptoms
Monitoring both the patient's progress and their reaction to treatment
Education of patients and members of their families regarding behavioral symptoms, medications, and management
Counseling for mental health issues provided in the client's home by a social worker, with the goal of enhancing coping, communication, and other skills.
Ensuring that the patient's primary care physician and the home health team are on the same page regarding their treatment
